A Safe Haven for Recovery in Baltimore — Where Healing Meets Home
Welcome to Lighthouse Behavioral Health Center at 1116 E 20th Street, a licensed 8-bed residential program offering care across ASAM Levels 3.1, 3.3, and 3.5. We provide structured, trauma-informed support in a warm, home-like setting where every individual is known, valued, and supported.
Our Story
Every person's journey is unique, but the need for dignity, compassion, and stability is universal.
Our roots began in supportive housing services, where our leadership saw a powerful truth: chronic homelessness and mental illness often go hand-in-hand. Too many people were slipping through the cracks.
In 2013, we began as a small practice dedicated to bridging this gap. Since then, we've grown into a full continuum of care — outpatient services, psychiatric rehabilitation, and now, safe and structured residential programs like our 1116 E 20th Street location. Our mission has never changed: to offer a place where people can stabilize, rediscover their strengths, and prepare for brighter tomorrows.
Our Vision
To be Baltimore’s most trusted community-based behavioral health provider, recognized for restoring hope, empowering independence, and building connections.
Our Mission
We provide safe, structured, and compassionate behavioral health services that support recovery, promote dignity, and foster community re-entry.
At Lighthouse, care is not just clinical — it’s personal. We don’t just see conditions; we see people, stories, and futures waiting to be rebuilt.
Addiction-Focused Residential Treatment With Immediate Access
Our residential program is designed specifically to help adults struggling with substance use disorders stabilize, recover, and rebuild their lives. We provide 24-hour, structured residential addiction treatment that combines clinical substance use care, mental health support, and a safe living environment with room and board and three meals daily. With rolling admissions and expedited intake, individuals can often begin substance use treatment the same day by calling or scheduling an intake online.
Rapid Substance Use & Mental Health Assessment
Every admission begins with a same-day substance use and mental health assessment, allowing our clinical team to quickly determine the appropriate ASAM level of care and create a personalized addiction treatment plan. This ensures treatment begins immediately with a clear focus on stabilization, relapse prevention, and long-term recovery.
Evidence-Based, Trauma-Informed Addiction Therapy
Clients receive structured individual, group, and family therapy focused specifically on substance use disorders and co-occurring mental health needs. All therapy is delivered using evidence-based practices within a trauma-informed environment that promotes accountability, emotional safety, and measurable progress.
Residential Living With Room, Board & 24-Hour Support
Clients live on-site in a supervised residential setting with room and board provided, including three meals daily. This 24-hour structured environment removes outside triggers and distractions, allowing clients to fully focus on recovery while receiving consistent support and monitoring.
Medication Support & MAT Coordination
When clinically appropriate, our team provides medication monitoring and coordination, including Medication-Assisted Treatment (MAT), to support withdrawal stabilization, reduce cravings, and improve treatment engagement throughout residential care.
Access to Detox & Specialized Services Through Coordinated Care
For individuals who require medical detox, psychiatric stabilization, or other specialized services, we coordinate expedited referrals and collaborative care with trusted partner providers—ensuring continuity of addiction treatment without gaps or delays.
Case Management, Peer Recovery & Life Skills Support
Each client is supported by dedicated case management and peer recovery specialists with lived experience. Clients also receive vocational and life-skills support to strengthen independence, accountability, and readiness for life after residential treatment.
ASAM Levels 3.1, 3.3 & 3.5 Addiction-Focused Residential Care
We provide a full continuum of residential addiction treatment, from low-intensity structured living (ASAM 3.1) to moderate and high-intensity residential care (ASAM 3.3 and 3.5). This allows clients to be placed at the right level of care and step up or down as clinical needs change.
With rolling admissions and expedited intake, most individuals can begin addiction treatment the same day.

A Message from Our Founder
"Lighthouse was created because we saw too many people struggling without the right support. Our goal was simple: build a program that combines the clinical strength of a licensed facility with the heart of a home. When you trust us with your care, you're not just a resident — you're family."
— Vernon Holmes, Founder
