Lighthouse provides behavioral health residential services (ASAM levels 3.1, 3.3, 3.5), supportive care, therapy and aftercare planning.

To be eligible, individuals must meet program criteria (clinical assessment, referral, safety considerations).

Referrals can come from hospitals, agencies, outpatient providers, families, etc.

An intake assessment will determine appropriateness, capacity, and safety.

Admission is subject to agreement in writing (resident / guardian) including service plan, rules, responsibilities.

Services may be covered by insurance, Medicaid, private pay — depending on facility policy.

Residents / responsible parties will be informed of cost, co-pays, or uncovered charges before service begins.

If insurance denies or fails to pay, resident may be responsible for payment in accordance with agreement.

5. Confidentiality & Privacy

Resident information is kept confidential in accordance with HIPAA (if applicable), state law, and professional standards.

Only staff or authorized referral partners involved in care or oversight will have access to protected health information.

Postal mail, phone calls, and visitation are subject to resident consent and scheduling.

6. Discharge & Termination

Discharge may occur when the resident’s needs exceed facility scope, or when resident voluntarily leaves.

Illegal behavior, violation of safety rules, non-payment or behaviors that jeopardize safety may lead to termination of services.

We commit to providing discharge planning and referrals to safer or less restrictive settings when possible.

7. Liabilities & Disclaimers

We provide care to the best of our abilities and under licensed regulations, but cannot guarantee outcomes.

We are not responsible for harm arising from failure to disclose medical or behavioral health history, or non-compliance with treatment plan.

8. Governing Law & Dispute Resolution
